🔬 What does the test measure?

The test analyses 2 key gases at 7 time points over 3 hours:

1. Hydrogen (H₂)

• Produced by bacteria during the fermentation of carbohydrates
• The most common type of SIBO
• Causes bloating, flatulence and diarrhoea
• High values indicate bacterial overgrowth

2. Methane (CH₄)

• Produced by archaea (microorganisms similar to bacteria)
• Associated with IMO (Intestinal Methanogen Overgrowth)
• Causes constipation and slow digestion
• High values indicate overgrowth of methanogens

🩺 What is SIBO?

SIBO (Small Intestinal Bacterial Overgrowth) is a condition in which bacteria grow excessively in the small intestine. In a healthy small intestine, there are relatively few bacteria compared to the large intestine. In SIBO, bacteria from the large intestine move into the small intestine or multiply there excessively.

How does SIBO develop?

Normal protective mechanisms:

• Stomach acid - kills bacteria
• Bile acids - have antibacterial effects
• Intestinal motility - "cleansing waves" remove bacteria
• Ileocecal valve - prevents the backflow of bacteria

Causes of SIBO: ❌ Low stomach acid - use of proton pump inhibitors (PPIs)
❌ Slow intestinal motility - after surgery, in diabetes, in hypothyroidism
❌ Structural problems - adhesions, strictures, diverticula
❌ Insufficient function of the ileocecal valve
❌ Excessive use of antibiotics - disruption of gut microflora
❌ Chronic stress - affects intestinal motility
❌ Age - reduced production of stomach acid

📋 How does the test work?

1. Order the test - we'll deliver a home test kit to you
2. Prepare - follow the preparatory diet 24-48 hours before the test
3. Take the baseline sample - breathe into the first bag (on an empty stomach)
4. Drink the lactulose - 15 ml of lactulose solution
5. Take 6 further samples - at set time intervals (20, 40, 60, 90, 120, 180 minutes)
6. Send the samples - in the enclosed prepaid envelope
7. Get your results - digital results within 10-15 working days with a detailed report
8. Consult - the option to discuss your results with an expert

⚠️ IMPORTANT PREPARATION BEFORE THE TEST:

For accurate results, it is ESSENTIAL to follow the preparation instructions:

4 WEEKS BEFORE THE TEST:

❌ No antibiotics - at least 4 weeks must have passed since finishing a course of antibiotics
❌ No bowel problems - wait 4 weeks after a colonoscopy, barium exam or diarrhoea

2 WEEKS BEFORE THE TEST:

❌ No probiotics - stop taking probiotics at least 2 weeks before the test

7 DAYS BEFORE THE TEST:

❌ No laxatives - do not take any laxatives
❌ No antacids - do not take medicines containing aluminium or magnesium hydroxide

48 HOURS BEFORE THE TEST:

Foods you CAN eat: ✅ Baked or fried chicken, fish or turkey (salt and pepper only)
✅ White bread (white only)
✅ Cooked white rice
✅ Eggs
✅ Clear chicken or beef broth (no vegetables)
✅ Tofu with salt and pepper (for vegetarians)

Drinks: ✅ Plain water
✅ Coffee and tea (no sugar, artificial sweeteners, milk or cream)

Foods you MUST NOT eat: ❌ No fibre (wholegrain products, legumes, cabbage)
❌ No sugar or sweets
❌ No alcohol

12 HOURS BEFORE THE TEST (FASTING):

❌ Eat NOTHING - only water is allowed
❌ No medication - except essential ones (consult your doctor)
❌ No chewing gum
❌ No sweets
❌ No mouthwash

1 HOUR BEFORE THE TEST:

❌ No smoking (not even passive)
❌ No physical exertion
❌ Do not sleep
❌ No toothpaste - just rinse your mouth with water

DURING THE TEST:

❌ Do not drink water after drinking the lactulose solution (for the first hour)
✅ 1 hour after drinking the lactulose, you can drink plain water

🔬 Types of SIBO:

There are different types of SIBO that require different treatment approaches:

1. Hydrogen SIBO (H₂-SIBO)

• The most common type
• High hydrogen levels
• Symptoms: diarrhoea, bloating, flatulence
• Bacteria ferment carbohydrates and produce hydrogen

2. Methane SIBO / IMO

• More accurate name: IMO (Intestinal Methanogen Overgrowth)
• High methane levels
• Symptoms: constipation, bloating, slow digestion
• Archaea (not bacteria) produce methane

3. Mixed type

• High levels of both hydrogen and methane
• Combination of symptoms
• The hardest to treat

4. Hydrogen sulphide SIBO (H₂S-SIBO)

• This test does not measure it
• Symptoms: diarrhoea, burning sensation, rotten-egg smell
• Requires a special test

💊 SIBO treatment:

After a positive test result, there are several treatment approaches:

1. Antibiotic treatment:

• Rifaximin - most commonly used, is not absorbed into the bloodstream
• Neomycin - in methane-dominant SIBO
• Metronidazole - for the mixed type
• Duration: 10-14 days

2. Herbal treatment:

• Oregano oil - strong antibacterial effects
• Berberine - reduces bacteria
• Allicin (from garlic) - a natural antibiotic
• Neem - antimicrobial effects
• Duration: 4-6 weeks

3. Elemental diet:

• A liquid diet with predefined nutrients
• Bacteria have nothing to digest
• Very effective, but demanding
• Duration: 2-3 weeks

4. Dietary adjustments:

• Low-FODMAP diet - reduces symptoms
• Specific Carbohydrate Diet (SCD)
• SIBO Bi-Phasic Diet
• Avoiding foods that feed bacteria

5. Prokinetic treatment:

• Supports gut motility
• Prevents recurrence
• Examples: Motilium, Iberogast, ginger

🔄 Preventing SIBO recurrence:

SIBO has a high recurrence rate (40-60% within 9 months), so it's important to:

❓ Frequently asked questions:

Why is lactulose used instead of glucose? Lactulose passes through the entire small intestine, which makes it possible to detect SIBO along its whole length. Glucose is absorbed earlier and may miss SIBO in the distal part.

Do I need to mix the lactulose with water? No, the lactulose should be drunk as it is. If you have trouble with this, you can dilute it with a little water - it won't affect the results.

Why are there holes in the plastic bag? That's correct - the bags have special valves that make them easier to fill.

Can I open the tubes? No! The tubes contain a vacuum and must not be opened.

What if I forget to take a sample at the right time? Try to follow the time intervals as precisely as possible (±5 minutes). If you miss a sample, make a note of it and continue with the rest.

Can I do the test if I'm taking medication? You can take most medications, but check with your doctor. Antibiotics and probiotics must be stopped 2-4 weeks before the test.

Are the results reliable? Yes! The SIBO breath test is the gold standard for diagnosing SIBO. We work with ISO-certified laboratories.
